Pricing
Practice activation is free, with no setup fee and no required sales call. Active patient care navigation is $49 per active patient per month, and billing begins only after a patient completes activation. No patient activated means no monthly patient fee. There is no minimum and no long-term contract.
Clinical decisions remain with the treating practice. Sidewalk supports care navigation and structured check-ins.
